AB Patientenzentrierte Kommunikation und selbstbestimmte Entscheidungen gelten heutzutage als wichtige Aspekte der Arzt-Patienten-Beziehung. Im klinischen Alltag ist die Umsetzung dieser Konzepte jedoch immer wieder erschwert. In diesem Beitrag wird anhand eines Fallbeispiels diskutiert, worin die Grenzen der partizipativen Entscheidungsfindung bestehen und wieso das Vertrauen in die Handlungslogik der Institution insbesondere dann relevant wird, wenn es nur geringe Spielräume für eigene Entscheidungen gibt.
AB Patient-centred communication and patient autonomy count nowadays as important ingredients of the doctor-patient relationship. However, clinical practice and working conditions are not always supportive to patient-oriented forms of communication. Drawing on a case study from HIV-counselling, this article discusses the scope and limits of shared decision making, arguing that trust and believe may be equally important, especially in situations with limited space for decisions.
